    • Licensed Program Manager: This position sits at the intersection of clinical oversight, program operations, and community-centered care. The Licensed Program Manager will provide clinical supervision required by DHCS under CalAIM for CHW, ECM, and all navigation/case management services, while leading the day-to-day management of LCC's community-based programs. This role is responsible for building and structuring LCC's integrated case management model, supervising program staff, reviewing and approving care plans, reporting and ensuring compliance across all CalAIM initiatives. The ideal candidate is deeply familiar with the Homelessness Prevention System (HPS), CalAIM/ECM frameworks, and the complexities of managing multiple community-based programs simultaneously. For more information and to apply, please click here
