HPS Intake & Assessment Specialist: The HPS Intake & Assessment Specialist helps vulnerable families in Santa Clara County stay housed by conducting eligibility screenings and assessments for households at risk of homelessness. As part of Latinas Contra Cancer's role in the county's Homelessness Prevention System network, this position meets with families to gather information, complete required paperwork, and evaluate homelessness risk using standardized tools and protocols. The Specialist determines program eligibility and lays the groundwork for effective case management and service delivery. Reflecting LCC's holistic, compassionate approach, this role recognizes that housing is one piece of a family's broader path toward stability and self-sufficiency, and carries a steady commitment to doing "whatever it takes" to help families not just avoid homelessness, but thrive.
Licensed Program Manager: This position sits at the intersection of clinical oversight, program operations, and community-centered care. The Licensed Program Manager will provide clinical supervision required by DHCS under CalAIM for CHW, ECM, and all navigation/case management services, while leading the day-to-day management of LCC's community-based programs. This role is responsible for building and structuring LCC's integrated case management model, supervising program staff, reviewing and approving care plans, reporting and ensuring compliance across all CalAIM initiatives. The ideal candidate is deeply familiar with the Homelessness Prevention System (HPS), CalAIM/ECM frameworks, and the complexities of managing multiple community-based programs simultaneously.
