Medi-Cal Contracted Services

Culturally Rooted Support for Your Health Journey

Latinas Contra Cancer is a proud Medi-Cal contracted provider of Community Health Worker (CHW) services, delivered through partnerships with Managed Care Plans. These services are designed to eliminate barriers to care and support Medi-Cal members with the greatest needs, particularly those who have been historically excluded or underserved by traditional healthcare systems.

Our CHWs, known across communities as promotoras, health educators, patient navigators, or advocates, are unlicensed, highly trained community leaders who walk alongside individuals facing chronic illness, housing instability, language or cultural barriers, behavioral health concerns, and more. CHWs are preventive care professionals. They help people take charge of their health before a crisis arises and ensure access to vital resources when one does.

At Latinas Contra Cancer, our CHWs are embedded in our Patient Navigation and Health Education programs, and they deliver trauma-informed, culturally responsive services rooted in the lived experiences of the communities we serve.

CHW services are free to eligible Medi-Cal members and support a broad range of health and wellness needs, including but not limited to:

  • Health Education

  • Health Navigation

  • Screening and Assessment — CHWs provide screening and assessments for health risks and social determinants of health, and link members to needed care or services.

  • Individual Support and Advocacy

Referrals to our CHW services can be made by your Managed Care Plan, primary care provider, or community-based organization. You can also contact us directly for support.
